It depends. If you want to let the player draw a few lines or basic shapes, then there are ways to do it pretty cleanly and efficiently. If you want full on drawing like Corel or MS Paint or something

You need some rule like "every 0 seconds" if mouse is down spawn actor X.

I use Gamesparks because that is what I know, it is very open and I have multiple free instances, but you can do what I am doing with any NOSQL/Mongo DB with some sort of javascript engine.
