Hello,
Could you, please give me some advices about the game icon?
And also, please answer these questions:
1. Do I have to make two icons (one for iPhone 4 and second for others)?
2. Do they need to be the same?
3. What resolution must they have?
4. I noticed that almost all app icons have rounded corners, do I have to make corners like these, or maybe Apple automatically crops them?
5. What image format it have to be? png?
6. Is it possible to make icon with alpha channels and it'll be displayed properly on devices?
7. Are there any other restrictions about the icons?

It should be a .png file saved at 72 dpi
When you upload to Apple, you will also need to provide a 512x512 icon. It should be identical or it might not get approved.
Apple will round the corners for you.
You cannot have alpha channels in your icon. If you do, Apple will fill it with black.
I was wondering - App icon in the app store is smaller than 114x114 pixels, so I guess that original icon is resized by Apple. But what about the icon in game site/description/card? It is a lot bigger than 114x114 pixels, so Apple also resized this icon? But in that case the icon might look blurry, right? And the 512x512 icon is only for Retina?
So, could anyone please explain to me, what about the icon in game site/description/card?

The 114x114 is used for the Retina display. Essentially it's double the old size (57x57) so it looks better on the retina display devices, and it doesn't look too shabby on older devices either
