app submission to app store advice

Can someone provide some advice on the icons used?

my app got rejected because the 57x57 icon and 512x512 icons were different. the reason was because it would confuse the user. The page quoted by them in the dev guide made no reference to the icons and the area in the document i did find which explains about the icons states to make the 512x512 one nice.
it also said "Please do not scale-up smaller artwork, as this could appear pixelated and blurry.
PNG image file formats are not acceptable in iTunes Connect for your Large Applica-
tion Icon."

So my question is:

1. what do people do for each icon? Same icon for both 512 and 57 or different?
2. Do you upload the 512 as png or jpeg?

Seems from doing a search on other forums, the rejection reasons can be so inconsistent that its pot luck that an app will get accepted.

    And they don't have to be the exact same, just instantly recognizable as the same app. My iGentleman app has a man's silhouette and app name as the icon, but my small icon is just that silhouette's upper body.
