Code signing failed - How to fix this?

Hey guys .
i have 6 apps in the store and iv'e never got this error before! what is this?

Here is the error when GS is Publishing:
Code signing failed. Make sure your provisioning profile,
developer certificate, and WWDR certificate are installed.

i followed TSB video to try and fix it:

in min 01:30 he say click on the "NEW" and "UPLOAD" the certificate request , even thought i dont know why should i do it , there is no such buttons in my certificate window nor does he has it in the video .
Can someone please help me here? im lost, i have absolutely no idea what to do (and i hate this feeling)

Not that im aware of expires in 2012.
i just made a new app id and new provisioning , and the publishing made with no errors, i have no idea what went wrong before .

New problem , i sent to application loader and i get an iPad error saying that the minimum version must be 3.2
So why there is the option of choosing 3.0 ?!?!?
God this is so frustrating , and now i try to publish again and GS tells my the server is down.

Yes i noticed that , ok now i sent to apple , that was an exhausting 2 hours tough .
