No, you don't have to copyright your name, It's kinda done when you put the app out.

Yes, you need a webpage for your app, but it can even be just your profile page here on GameSalad.

As for a website - a definite no, you don't have to have a website to release your app! It's up to you if you think it will generate more sales, then go for it.

You don't have to MAKE a website, but you have to have a link to a developer website, or developer support page, which in both cases you can use your GameSalad profile page.

EDIT: It really would be best if you did have a website for your app development.

I'm putting the finishing touches on a game I was planning on calling island quest but I just found out there's another mobile game (not on the app store) made for nokia with that name. If I changed the name to Island Quests then I shouldn't have to worry about trademark infringement, right?

Titles aren't copyrightable at all. I could write and sell a novel called Moby Dick if I wanted to, it could even be about an old sailor trying to kill a white whale, but as long as I'm not copying the actual "content" of Melviles book, then it's all ok. I also believe you can't copyright recipes, either. Too generic.
