I have a simple question for native english speakers.
What is the correct form for the title of game : "The Witch" or "Witch" ?

    Either way would be ok in my opinion. The word "The" is not necessary. It is only noticed as being missing when there is a sentence, not in a short title. There are plenty of titles without the word "The" that are fine. "Asteroids"... "Adventure"... "Aliens" ... "Plants vs. Zombies". Either way would be fine.
