app was approved, but how to complete the process

My app was approved today, but it is saying i need to complete the bank management contracts as well as the W-9 tax forms. I do not have a company and want to know how to fill these parts out in itunes connect. any help would be great. Thanks

    log into itunes connect, go into contracts,tax and banking and fill the info out as well as request the ios paid contract.

    You should have done that before submitting your app btw, your paid apps wont appear in the appstore till all that is done and processed

    Yes I know that. But for the tax form do I select payee exempt or subject to backup witholding?

    that all depends if you want apple to hold the taxes for you, or not hold anythign and you declare the taxes yourself. i think where you live has a factor in that too.

    Im in the us and have payee exempt, and i file my own taxes. But i have heard other members in Canada choose subject to backup withholding and then apple holds the taxes im pretty sure.
