Where Can I find the App Rating Template from the Video

I watched the App Rating Process for Pro users video a few times and am wondering where I can find the template he is using? He says something about the tooler on the website, not sure where that is? I am a pro user.

    Nevermind... I found it after searching high and low on Google, it is actually a template built into the GameSalad software if anyone else needs it.
