Picture with Holes?

in Working with GS (Mac)
Is it possible to put a .png picture in to a app (or any other picture type) and have a hold in it so something could go through it? I am making an endless game like falldown but I am having troubles with randomly generating.

    You considered using different layers and having the image over lap a copy of the original image with transparency
