Did you try deleting the old .apk on your phone first and reinstalled?
I have never tried installing through internal storage, probably don't know how.
Always tested with GS viewer and when it works i upload the .apk through mediafire and then on my phone installed from there.

Are you testing adhoc or through the GS viewer?
I just connect via usb and copy and paste into the phone's internal storage
Did you lock the screen orientation before you published from the GameSalad creator?
sounds like you didn't
uncheck scene->autorotate->landscape
checked scene->autorotate->portrait

In your project, inside Game Salad
go to your scene you'll see autorotate attributes
You should also use the GS android viewer too, it'll save you some time testing whenever you makes changes.
