ipad and iphone versions

Ive designed my current project with ipad in mind, and will then copy the project and resize for iphone. what im wondering, is do these have to be uploaded as separate apps to appstore - do they need different names etc (to avoid ipad users downloading the wrong version)
- what do others usually do in order to have both ipad and iphone\touch versions?
