Open iTunes at top of screen it says file click that and select add to library and locate your file and then it load into your apps. then you will have the option to install on your device in the apps list.
oh once you select install sync device then it installs.

I just keep XCode's Organizer window open. (XCode -> Window -> Organizer)
If my device is attached then I can just drag my adhoc file into my Applications folder of the device and viola.
